Concussion Code of Conduct

Attention ALL PARENTS and PLAYERS!

Below you will find a mandatory Concussion Code of Conduct that needs to be reviewed by both player and parent! Once you have reviewed the code of conduct there is an acknowledgement form that needs to be completed and signed, and scanned back to Tammy Egli, our Registrar (do not take a picture of these forms as they have to be uploaded to the participant files).

If you do not have a scanner you can drop the forms to Tammi at 2065 Line 47 (the Lisbon Road out side of Wellesley).  There is a blue box on the porch. These form's must be completed and returned by September 8th. Your child will not be permitted on the ice without the completed acknowledgement form.  Tammi will offer two sessions where you can come and review the Concussion code of conduct and sign the acknowledgement form. Please remember both the parent and child must be in attendance to sign and review the code of conduct -

Wellesley Arena           Monday  Aug 26th 6-8 
St. Clements Arena      Tuesday Aug 27th 6-8
